It is an operation that is performed as a treatment for morbid obesity. The truth is that when talking about stomach reduction we are talking about any obesity intervention in which the size of the stomach is reduced.

Of all the interventions that are made to solve the morbid obesity, the most performed is the Gastric bypass

Depending on each case and depending on the operation of obesity operations, there are three types of stomach reduction or surgery to control obesity.

PIPE OR GASTRIC SLEEVE

It is the least invasive of the techniques and the one that after the intervention has less side effects and allows a better quality of life of the patients.

It consists of the reduction of the stomach, removing the pocket that allows it to expand and have more capacity, therefore it does not affect the noble parts of the stomach and therefore the functionality of the stomach is not affected.

This type of surgery allows a normal life of 7 days, since surgery is performed laparoscopically and with minimally invasive anesthesia, being only admitted to the hospital for 24 hours.

Gastric Plicature

This innovative technique consists in applying the greater curvature of the stomach, and in this way, its capacity is reduced by 80%

 

The surgery is performed by laparoscopy, so it is minimally invasive. It consists of suturing the left edge of the stomach on itself to create folds that occupy the inside of the stomach and thus decrease the volume of it. The stomach will no longer admit the same volume of food as before and satiety will appear much earlier.

Being a novel treatment we cannot have many testimonies, but most patients lose between 50 and 60% of their excess weight during the first year after surgery. As with other weight loss procedures, diseases related to obesity can also improve.

Unlike gastric sleeve and gastric bypass surgery, there is no stapling of the stomach or intestine. No part of the stomach or intestine is cut or removed. Theoretically, the gastric plication can be reversed but probably in most cases, it is more difficult to reverse than the gastric band. It can also be converted to another procedure if necessary.

This technique would be indicated in patients with Body Mass Index (BMI) above 40, or above 35 if they also have diseases associated with obesity (hypertension, diabetes, …)

 

The completion of a gastric plication lasts from one to two hours. The hospitalization is of 24-48 hours and a significant percentage of patients will present nausea and vomiting during the first days.

Recovery begins almost immediately after surgery as you begin to learn a new way of eating. The initial diet includes only liquids for two weeks. The following weeks will be introduced thicker liquids until the solid is about a month after surgery.

 

Gastric plication does not cause alterations in the absorption of vitamins and minerals, but as in the rest of restrictive techniques, a vitamin supplement is recommended since sometimes the intake of less food may cause a decrease in the supply of vitamins and minerals.

Gastric Bypass and classic roux and the new type sadis bypass technique

The gastric bypass is the most contrasted and known technique for the treatment of obesity. The Sadis Bypass technique is a new technique derived from the previous one, but with this method, the more physiological bypass is achieved, with fewer adverse effects, and therefore, the quality of life of patients who undergo this type of treatment. Gastric bypass is much better, with less need to take vitamin supplements.

The technique used in Spain includes:

  • The invaginating plication of all the section lines with endogenies or endo-staplers, minimizing the incidence of postoperative leaks or bleeding.
  • The manual preparation (using stitches) of the anastomosis or union between the duodenum and the ileum (Bypass type SADI-S) or the stomach and ileum (classic gastric bypass), decreasing to almost zero, the possible incidence of leaks postoperative.
  • Up to now, there has been no leakage in this Bypass technique.
